Our outreach work with clients in mental health rehabilitation provides social guidance to people in mental health rehabilitation who live at home, in their own rental or owned housing. We use digital tools in our work.
We support the daily life and life management of people in mental health recovery through remote meetings and home visits. You can use your own smart device to attend remote meetings, but you can borrow a tablet device from us, if necessary. We set the goals for our collaboration together.
How do I become a client of the outreach service?
Apply for the service together with a social or health care professional who is familiar with your situation. The application includes an assessment and clarification of the need for the service. This includes information on how your physical, mental, cognitive, and social functioning capacities have been assessed, maintained and promoted, as well as your possibilities for rehabilitation.
- Apply for the service by submitting the form ‘Assessment of the need for housing services’.
- Send the completed application form by mail to Housing Support, PO Box 8660, 00099 City of Helsinki (Toinen linja 4H).
Your application will be processed by the Assessment Manager for Housing Support. The decision will be made by a working group. We will contact you when a decision has been made.
